For referring doctors
What the review is, and how to refer a patient for it.
Refer with a brief letter to Dr Praveen Jayarajan at Medical One QV, Level 3, 23 QV Terrace, 292 Swanston Street, Melbourne 3000. Bookings are by phone only: the patient calls 03 8663 7000 and asks for a 45-minute appointment for a men's health check. Recent results and specialist letters sent with the referral, ahead of the appointment, are scanned into the patient's file before they're seen.
The patient is seen for a 45-minute preventive review; investigations are ordered only where there is a documented indication, and everything ordered is followed up. You receive the results, the plan and a one-page summary. Patients return to you for ongoing care. This is not a general health check in the Cochrane sense; investigations are Red Book items with an indication, plus a few declared additions, each with its guideline on the tests page. The review's job is to make sure what's indicated gets done. It isn't a panel.
